"The relationship between the Practice and patient is a partnership and as such both parties have responsibilities to each other."

To apply for registration with the Practice, please bring your NHS card to the reception. We do not transfer patients between Doctors in the Practice. You are able to see the Doctor of your choice regardless of which Doctor you are registered with in the Practice.

The doctors do not run personal lists.

If you move house, please inform us of your new address. It may be necessary to register with a Practice nearer to your new home.

Please remember that if you are dissatisfied with the service given by us, it is in your right to transfer to another Practice. Avon Primary Care Support Agency will allocate you a Practice if you cannot find another Practice who will accept you onto their list.

The Doctors are entitled to remove a patient from the Practice List if the partnership between the Practice and patient is not working.

We encourage everybody who joins the Surgery to have a check with the Practice Nurse, which includes blood pressure, weight, medical history and health promotions. This helps us get to know you and is an opportunity for general health advice. You may also request general health checkups.
